The Case Management Department at Flowers Hospital consists of Registered Nurse Case Managers and a Case Management Coordinator. The Case Manager’s role is to work closely with each patient’s physicians, nurses, rehabilitation services, dietary, other healthcare members, insurance companies and community resource agencies to facilitate and coordinate the appropriate plan of care to best meet the patient’s needs.
The Case Manager will assist with arranging post discharge needs to include Home Health care services, durable medical equipment, skilled nursing home placement, rehabilitation placement and identifying other community resources. A Case Manager is available seven days a week with someone on call after hours to assist.
