Discharge Process

Preparations for discharge begin when you are admitted to Flowers Hospital. The Social Services Department staff can assist in making arrangments for post-discharge needs, including home health care servcies, in-home medical equipment, temporary or long-term nursing facility placement, as well as assisting with arrangements for other community services. If you have concerns or would like to discuss other discharge planning options, ask your nurse to have one of the Social Services staff visit you, or you may call (334) 794-5000, extension 1148.
